These templates are the product of many hours of research and editing. They have been reviewed for federal Title IV-E and ASFA compliance by DCFS staff. The templates are presented in Word format, which you can easily download. Text boxes allow for “reasonable efforts” and “best interest/contrary to the welfare” individualized findings and for the customization and expansion of other court findings and orders.
